Drug & Alcohol Evaluation Package

If you’ve been ordered by a court, judge, probation officer, or other authority to complete a drug and alcohol evaluation, Connect Counseling can help. We provide comprehensive evaluations that work to meet legal requirements—and we approach the process with respect and without judgment.

What’s Included

Comprehensive Assessment (3-4 clinical evaluation hours across multiple sessions):

• Detailed substance use history (what, when, how much, patterns, consequences)
• Medical, psychological, and family history as it relates to use
• Trauma screening and mental health assessment
• Social/relational factors and support systems
• Assessment of where you stand in your readiness for change
• Written letter with findings and recommendations

The Letter. A detailed, professional letter suitable for court, probation, or other legal proceedings. Includes summary of your history, counseling findings, diagnosis (if applicable), level-of-care recommendations, and specific recommendations for counseling or monitoring.

Debriefing of Findings. We’ll review the letter and our findings with you so you understand what it means for you before we submit it.

Timeline. Allow 1-2 weeks from your first session to letter completion. An expedited process may incur an extra charge—ask about this when you call to schedule.

How to Schedule

Call Michael Hoober at 717.209.2357 and let him know you need a court-ordered evaluation. He’ll confirm we can help, answer questions, schedule your first appointment, and explain what to bring.

What to Bring: Photo ID; any court documents or orders; insurance card (if applicable); list of medications; information about any previous counseling or treatment.

Cost

D&A Evaluation Package: $750. Includes 3-4 clinical evaluation hours plus a written letter. Payment is due at your first appointment and is self-pay. We accept all major payment methods, and we can provide a superbill if you’d like to seek out-of-network reimbursement from your insurance.

What Happens After

The evaluation is complete once the letter is delivered. However, it may recommend ongoing individual counseling, family counseling, or a higher level of care. If that makes sense, we can discuss it separately. This is entirely your choice.

Important to Know

• We are not here to judge. We’re counselors trained to assess substance use within the full context of your life.
• The evaluation is confidential except as legally authorized by you. We will not disclose findings beyond what’s legally authorized.
• You have rights. You’re entitled to keep a copy of your evaluation letter.
